This report calculates how many times per month a student received services per practitioner for a specified range of months (max 12 months) at a specified school.  The results are displayed in a in a grid format.  The total counts per student are shown for the specified monthly range as well as a breakdown per month within the range.  If the user is logged on to the district office, the district totals will be reported on the last page.

Service Tracker Monthly Service Summary Report is located within Reports in the Programs menu in Q.

Report Options

The ST Monthly Service Summary report offers the following options:

  • Title – A default title is provided but may be changed as desired.
  • School – Only selectable when logged into the District Track and/or when the District Preference to ‘Include Students from All Schools’ is ON. If the preference is OFF and the user is logged into a school, only the logged in school will appear in the School selection field.
  • Include Students:
    • Serviced by Me – Students included in the report are limited to those on the Caseload of the Practitioner when the Practitioner does not have Service Tracker Administrative permissions.
    • Serviced by Selected Practitioners – this option appears only when the user has Service Tracker Administrator permissions. When selected, the user may then select all or specific practitioners.

  • Service Records – Select the service month/year date range for the report for a maximum of 12 months.

 Include – one of the following:

    • Only Students WITH Services Performed (default)
    • Only Eligible Students NOT serviced
    • Students WITH Services Performed and Eligible Students NOT serviced
    • No Students, Only Service Summary Counts

 Sort – Select either sort by

    • School, Practitioner, Student or
    • Practitioner, School, Student
  • Print Options
    • Page Break on Group Change
