This report prints the Immunization Status codes assigned to each student for a selected Immunization Rule.

Immunization Status Report is located within Reports in the Health menu in Q.

Report Options

 

The Q Immunization Status Report offers the following report options:

  • Title: Displays at the top of the printed report. Rename here if necessary.
  • Immunization Rule
    • Included Rule: Select an Immunization Rule to filter the report results by. Defaulted to ALL. CTRL-Click to select multiple Rules.
    • Rule Start Date: Defaulted to be blank, enter a date range to further filter the included immunization rules. The date range filters students by when their immunization rule was applied. The report will return all students whose rule began between these user specified dates.
  • Immunization Status
    • Included Status: Select an Immunization Status to filter the report results by. Defaulted to ALL. CTRL-Click to select multiple Statuses.
    • Status As Of: Defaulted to be the start of the term to today, enter a date range to further filter the included immunization status.
  • Immunization Exemptions: Show: Select to include All Students, only students with an Exemption, or only students without an exemption. Selecting Students with an Exemption will bring up a new selection field with Exemption Reasons. You may select from these to filter by a certain exemption.
  • Group Listing by: Choose between Grade, Immunization Rule, and Immunization Status. The report will group students based on this selection.
  • Print Options
    • Page Break on Group Change: If checked, the report will jump to a new page when the group changes. This only takes effect if you’ve selected something in the Group Listing by section.
    • Alternate Row Shading: If checked, the report will print shading between each section.
